Health Insurance

Employees permanently employed for twenty (20) or more hours per week are eligible for health insurance through the town of Lunenburg.  The town offers employees a choice between two Blue Cross/Blue Shield plans—HMO Blue and a Blue Care Elect Preferred.  New employees, who are eligible, may enroll in one of the plans at the time of hire.  Enrollment or changes in health insurance plans after the time of hire are only allowed during the open enrollment period (open enrollment period is announced by the town) except in cases of event-driven circumstances (a spouse’s loss of insurance coverage, a death, a marriage, a divorce, or a child birth/adoption).

 

Health Insurance Responsibility Disclosure (HIRD)

Chapter 58 of the Acts of 2006, An Act Providing Access to Affordable, Quality, Accountable Health Care requires applicable Massachusetts employers to file an Employer HIRD report annually. In order to do so, the town/school department must collect a HIRD form (Appendix E) from employees who either decline the town health insurance or subscribe, but decline Section 125 Plan participation. Employees will be asked to complete a HIRD form upon hire and again within the three-year cycle.

 

Dental Insurance 

The town of Lunenburg offers eligible employees a voluntary dental benefit through Altus Dental.  Employees are responsible for the full cost of the premium; however, the payroll deduction is taken on a pre-tax basis.  Employees can access this benefit during the open enrollment period announced annually by the town.  For more information please contact Nancy Forest at town hall or visit the Altus Dental website (www.altusdental.com). 

 

 

COBRA

The COBRA (Consolidated Omnibus Budget Reconciliation Act) health benefit became law in 1986 “to provide continuation of employer-sponsored group health coverage that otherwise might be terminated.”  Health Insurance participants will be given information regarding COBRA upon termination.
